About Us:
Hospice strives to provide people who are terminally ill with physical, emotional and spiritual support, to die with dignity, as free from pain as possible, and in the comfort of their own homes or at GVH’s Senior Care Center.Hospice also offers emotional support to family and friends during the dying process, as well as continued bereavement care. We are a small program providing personalized care to the community, however we are not staffed to provide 24/7 caregiving. Limited private pay caregiving is available through community members.
- 24/7 on call coverage RN case manager who coordinates care in conjunction with patient’s primary care physician Patient and family education to assist in making informed choices
- RN visits are determined by an interdisciplinary team (IDT). RN visits may be more frequent with symptom exacerbations or during dying process
- RN visits determined by nursing assessment with physician coordination
- Physical & Occupational Therapy
- Medical Social Worker consultation
- Weekly assessment phone calls by an RN Access to spiritual support Access to volunteer program as available
- Nurse Aide Services
- Medication, medical supplies, labs and other care equipment related to the patient’s terminal diagnosis are paid for by Hospice
- Hospice Lending Library
- Durable Medical Equipment Loans
